Icon disappearing

Once a day app icon seems to disappear from my homescreen and i have to search it from "all apps" Device problem rather than app issue? Tips how to fix it? Quite annoying especially when app don't seem to work too good atm with blackscreen and random crash i had too yesterday. Tried reinstalling but no good.
